Apparently some combination of Varimathres, Putress (Grand Apocathary) and/or Sylvannis just straight up give everyone the bird and unleash a new indiscriminate plague on everyone at the Wrath Gate. So then you go see Thrall and what looks like right now from Quest descriptions, go assault the Undercity itself.
Looks like Saurfang's son die and possibly Thrall =(
